Dear Parents,

 

All of us at Milburn, invite you to celebrate your child’s birthday by joining our Birthday Book Club and purchasing a book in your child’s honor. There are two ways to join the Birthday Book Club. One, for a donation of $5.00, your child will have the opportunity to choose a book that is new to our collection. Two, you can donate an appropriate new or gently used hard back book to the library in honor of your child. 

 

A bookplate will be place in the book. Your child will be the first student to check out the book, and when it is returned, it will be placed into circulation for all to enjoy.

 

If you are interested in participating, simply complete the form below and send it to school along with the book for donation or a check for $5.00 made out to Milburn School Library. If you have any questions, please feel free to contact me any time!
